Dialysis - Hemodialysis RN, Decatur

     JOB SUMMARY: 
  • The staff registered nurse will administer quality professional nursing care in accordance with facility policies and procedures while working collaboratively and communicating with members of the interdisciplinary team. 
  • Coordinates and delivers care through the nursing process to adult chronic hemodialysis patients utilizing standards of care set by the American Nephrology Nurses Association. 
  • Works to ensure the attainment of patient outcomes that meet KDOQI goals while following regulatory requirements of the Texas Department of State Health Services and the Centers for Medicare/Medicaid Services. 
  • Supervises Licensed Vocational Nurses and Patient Care Technicians while assuming a ‘hands on role’ and fostering a team approach to patient care delivery. 
  • Read, speak, write and understand English.
 
REPORTING RELATIONSHIPS: 
  • Reports to the Clinical Manager, Charge Nurse, and Director of Dialysis. 
  • Supervises LVNs and PCTs.
 
QUALIFICATIONS
  • 1) Current licensure to practice nursing in the state of Texas. 
  • 2)  Current CPR 
  • 3)  Normal color perception 
  • 4)  Minimum of 6 months of experience in acute or chronic hemodialysis obtained within the past 12 months preferred
  • 5)  Advance nephrology nursing certification (CDN, CNN, CHN) preferred.
 
Must have current licensure/certifications upon hire. 

New Hires will have 60 days to acquire the required Basic Life Support (CPR, ACLS, PALS, BLS, and/or any other life support certifications required for the job).
 
ESSENTIAL PHYSICAL DEMANDS AND WORK ENVIRONMENT: 
  • Requires walking, standing, stooping and bending. 
  • Must be able to do lifting of at least 20 lbs. 
  • Requires close mental, visual and auditory attention to patient. 
  • Work environment is characteristic of a health care facility with air temperature control and moderate noise levels.

  1. Consistently provides customer service as it applies to all internal and external customers, including patients and families, coworkers and physicians.

  3. Completes a minimum of 5 hours of renal-related education annually.
  4. Maintains a professional appearance and practices good grooming habits while adhering to the facility dress code.
  5. Consistently meets facility attendance standards.
Flexibility:
  1. Demonstrates flexibility in response to unexpected changes to work volume, acuity, staffing and scheduling.
  2. Completes other duties as assigned.
Patient care; planning, implementation and evaluation:
  1. Communicates accurate and pertinent information to the physician in a timely manner.
  2. Assesses and documents patient condition pre and post dialysis treatment or when indicated by a change in patient status.
  3. Completes a nursing assessment of patients prior to the initiation of their initial dialysis treatment in the facility.
  4. Completes the comprehensive patient assessment and care planning timely within 30 days of admission, at 90 days, and annually.
  5. Participates in the identification of unstable patient for purposes of conducting reassessments and care plan adjustments monthly.
  6. Ensures plan of care is individualized to reduce complications and promote health.
  7. Provides education for patient/family based on assessment of learning needs.
  8. Demonstrates competence in the initiation, monitoring and termination of hemodialysis treatments.
  9. Evaluates patient responsiveness to interventions in a timely manner.
  10. Documents evaluation and implements changes to plan of care collaborating with other disciplines as needed.
  11. Administers medications in accordance with facility policies and procedures.
Organization and Problem Solving:
  1. Participates in the patient admission process.
  2. Consistently documents daily treatment and medication charges for billing.
  3. Participates in the evaluation of skills and knowledge of unit personnel. 
  4. Organizes and prioritizes work to utilize resources wisely.
  5. Assists Charge Nurse with medication dose changes according to protocols as assigned.
  6. Notes, signs and implements physician orders accurately within a 48 hour time frame.
  7. Completes assignments in a timely manner minimizing overtime.
  8. Addresses patient care problems and concerns with practical solutions.
Leadership and Teamwork:
  1. Consistently demonstrates leadership and sensitivity toward coworkers by assisting as needed.
  2. Participates in orientation and development of new personnel.
  3. Consistently demonstrates respect of individuals and cultural beliefs and lifestyles of patients/family members and coworkers.
  4. Participates in the Quality Assessment Performance Improvement (QAPI) process through conducting audits and data collection as requested and making recommendations on quality improvement strategies.
  5. Maintains open communication to assure that the Clinical Manager and Charge Nurse are advised of issues and concerns.
  6. Oversees completion of clinic specific tasks as assigned.
Safety:
  1. Consistently provides for patient safety needs and identifies risk factors and age-appropriate safety measures.
  2. Practices good body mechanics and correct methods of moving, lifting and transporting patients.
  3. Assures a clean, safe environment at all times.
  4. Consistently assures that equipment functions properly prior to and during patient use. 
  5. Promptly reports any equipment problems or physical plant maintenance to the appropriate personnel.
  6. Consistently monitors effects of fluid removal and assesses dry weight, making recommendations to the physician based on assessment.
  7. Properly prepares, labels, administers medications and special dialysate baths, as permitted by facility policy.
  8. Adheres to all infection control policies.
  9. Consistently notifies personnel of potential hazards.
 
OSHA TASK CATEGORY TWO: 
  • There is inherent potential of exposure to mucous membranes or skin contact with blood, body fluids, or tissues, or a potential for spills or splashed during the performance of the job. 
  • Universal precautions must be observed and protective devices worn by the employee when performing a task that is likely to cause exposure.
 
Employment standards include current requirements as determined by Federal, State, and Wise Health System policy.
