  • The Palliative Care RN provides direct patient care in a variety of settings (i.e., private home, nursing home, hospital, clinic setting, TeleHealth).
  • The Palliative Care RN has the responsibility of patient and family education regarding end of life care, comfort care, and/or hospice treatment plans.
  • The Palliative Care RN, alongside the 360 Care Management Director, has primary responsibility of creating and implementing a new Palliative Care Program at WHS/WHC.
  • Knowledge of palliative care with emphasis on symptom control associated with terminal illness, knowledge of the holistic approach toward patient care, and acceptance of hospice philosophy and principles of care are expected from the Palliative Care RN.
  • The Palliative Care RN must work with all WHC facilities as well as post-acute agencies such as home health, skilled facilities, and hospice agencies.
  • The Palliative Care RN will conduct Chronic Care Management services from an outpatient setting.
Additional Responsibilities:
  • 1) Knowledge of  approved clinical guidelines and evidence-based protocols when providing care
  • 2) Performs comprehensive and focused evaluation of patients
  • 3) Provides services related to health maintenance and promotion, palliative care, and end of life care
  • 4) Collaborates with home health nurse, hospital nurse, Hospitalists and other physicians as needed to discuss identified patient needs, treatments, and other care
  • 5) Informs primary physician of services provided and collaborates with other physicians as needed; provides written reports to primary physician upon request
  • 6) Prepares and maintains accurate patient records, charts, and documents to support sound medical practice
  • 7) Provides training on palliative care topics for staff and students as needed
  • 8) Performs other duties as requested
  • 9) Adheres to the practice of confidentiality regarding patients, families, staff, and the organization
  • 10) Utilizes and models excellent customer service skills at all times, seeks opportunities to assist patients, families and co-workers; demonstrates teamwork and cooperation.
  • 11) Accepts direction and follows instructions from supervisor; seeks additional information as needed
  • 12) Adheres to all organizational and departmental policies and procedures
  • 13) Attends all required meetings and in-services; seeks opportunities for additional professional development activities as appropriate.
  • 14) Expands Palliative Care education to the community via classes, health fairs, and community health improvement projects
  • 360 Care Management Director operationally
  • 1)  Licensed to practice as RN in Texas 
  • 2)  CPR certified 
  • 3) Previous experience is preferred in Geriatrics, ICU, or Hospice care
  • 4) Palliative Care Board Certification preferred, expected to obtain within 6 months of hire date
Must have current licensure/certifications upon hire. 

New Hires will have 30 days to acquire the required Basic Life Support (CPR, ACLS, PALS, BLS, and/or any other life support certifications required for the job).
  • There is an inherent potential for mucous membrane or skin contact with blood, body fluids, or tissues, or a potential for spills splashes during the performance of the job. 
  • Universal precautions must be observed and protective devices worn by the employee when performing those tasks likely to cause exposure.
Employment standards include current requirements as determined by Federal, State, and Wise Health System policy.
