SUMMARY OF RESPONSIBILITIES:
  • The Manager – DSRIP and Community Health Improvement will serve as the program manager for the 1115 Waiver and population and community health initiatives to include performance in DSRIP and Community Health Improvement.
  • This position will monitor and report for programs identified by the CTO and will serve as the liaison to both the community and personnel within Wise Health Clinics and Wise Health System.
  • This position will ensure our compliance in the 1115 Waiver Program.
  • Working with leaders and stakeholders within Wise County to achieve improvement in outcome measures which will be assess through a Community Health Needs Assessment will also be a responsibility of this position.
  • This position will assist the Chief Transformation Officer in support of organizational transformation in adoption of Baldrige criteria and serve on the Transformation Team supporting organizational changes.
 
REPORTING RELATIONSHIP:  Reports to the CTO/Sr VP Strategic Alignment
 
REQUIREMENTS:
  • Bachelor’s Degree in Business or Healthcare Management
  • At least 2 years experience working on 1115 Waiver
  • Masters in Healthcare Administration strongly preferred
 
OSHA TASK CATEGORY THREE:  There is no anticipated risk of exposure to mucous membranes or skin contact with blood, body fluids, or tissues, or a potential for spills or splashed during the performance of the job.  Universal precautions must be observed and protective devices worn by the employee when performing a task that is likely to cause exposure.
 
Employment standards include current requirements as determined by Federal, State, and Wise Health System policy.
 
PHYSICAL REQUIREMENTS:  Sitting at desk and reading for long periods of time; intermittent walking, and standing; some bending, stooping, and reaching with lifting of some items up to a weight of 40 pounds.
